This form should be completed by the participant’s parent or guardian. If more than one program has been booked, please complete a form for each one.

Eligibility 

NGV Creativity Club is funded by the Department of Education, Victoria, through the Victorian Challenge and Enrichment Series (VCES). VCES is designed for high-ability students, to offer them enrichment and extension outside of their normal classroom setting.

It is available for Victorian Government School students only. If the participant attends a non-government school (i.e. a catholic school or an independent school, sometimes known as private schools) they are not eligible for this program. 

The participant must be achieving above standard in a relevant subject area (e.g. Visual Arts, English).

I confirm the participant attends a Victorian Government school and is Above Standard in at least one relevant school subject and therefore eligible to attend NGV Creativity Club 
If you have booked across multiple programs please complete a separate form for each one.

 I have read and agree with the following conditions:

NGV Creativity Club workshops are supervised by NGV Learn staff at appropriate adult/child ratios; therefore, parents are not required to attend the session (unless acting as a companion with a valid government issued Companion Card). 
Students will remain in the program for the duration of the day and must be signed in and out by a parent/guardian.
Students will need to provide their own packed lunch for this event. Please note this is a nut free event.
Students will be asked to complete a short anonymous survey at the end of the program.

Transport Subsidy

With the introduction of the Youth Myki, public transport is free for all under-18s. Parents and carers may be eligible to receive assistance with transport costs if they will be travelling from outside the Metropolitan Melbourne train network. This will take the form of a prepaid myki and/or pre-paid petrol voucher at NGV's discretion.
